2.) I need the exact placement information of each photograph; i.e: Magazine Name & Date of publication, or News article & date of publication, etc. Please send me a copy sheet, if possible.
3.) All Rights for the Leasing of Photograph(s) will be for a One-Time period not to exceed 3 Months.
4.) You may request usage of up to 3 Photographs during a 3 Month Period.
5.) I will need Your Name, Email Address and Daytime Phone Number.
